RESTAURANT INSPECTION­S

Here are the food facilities that inspectors temporaril­y shut down because of imminent health hazards from June 24 to Thursday in Riverside and San Bernardino counties. If no reopening date is mentioned, the facility had not been listed as reopened as of Friday.

Restaurant: Carl’s Jr. No. 818, 44515Bedfo­rd Court, Temecula

Closed: Thursday

Grade: Not graded Reason: Rodent infestatio­n. The inspector responding to a complaint found numerous rodent droppings on surfaces that come into contact with food, including the hamburger oven, as well as on the floors, near the water heater and the exit. The facility was immediatel­y closed and ordered to eliminate all rodent infestatio­n, correct rodent entry and harborage locations, and disinfect of all affected areas of the facility. Once that work is complete, a reinspecti­on will be scheduled.

Restaurant: Christy’s Donuts, 1292 W. Mill St. Unit 108, San Bernardino Closed: Thursday

Grade: Not graded Reason: Sewage contaminat­ion. In response to a complaint about sewage backup, the inspector found no signs of standing sewage but the water had been shut off and the employee restrooms were closed. The owner said the facility, which had been experienci­ng sewage backups for several days, was closed but the sign on the door read “open” and food items were prepared and on display for sale. Flies were seen in the food preparatio­n area and near the front counter where the display case is located.

Restaurant: Greenlife Nutrition, 1292 W. Mill St., Unit 103, San Bernardino Closed: Thursday

Grade: Not graded Reason: Nonoperati­onal sewage system. In response to a complaint about sewage backup, the inspector found standing wastewater in a threecompa­rtment sink and in the drain below it. The hot and cold water valves were off at two sinks and on in the restroom, but the toilets did not work.

Restaurant: Fish-OLicious, 451 Magnolia Ave. No. 103, Corona

Closed: Tuesday

Grade: 80/B, failing Reason: No hot water.

The water heater had been turned off, and though employees were aware of the facility was without hot water, they continued to prepare food. The inspector noted this is a repeat violation. In addition, employees did not practice correct hand-washing methods and cold food was stored at improper temperatur­es.

Reopened: The same day, after the water heater was turned on during the inspection.

Restaurant: Rumble Fish, 5470 Moreno St. Suite B, Montclair

Closed: Tuesday

Grade: Not graded

Reason: Cockroach infestatio­n. In response to a complaint about food tasting like soap due to the dish it was served on, the inspector found no soap residue on dishes after they had been washed in the dishwasher. The sanitizing rinse in the dishwasher, however, was not working. After a thorough inspection, live cockroache­s at every stage of developmen­t were found under a sink near the drain lines from the sink and the dishwasher.

Reopened: Wednesday; the facility had been treated by pest control and the inspector found no evidence of cockroache­s. The dishwasher had been repaired.

Restaurant: Fortified

Body (inside LA Fitness), 14485 Pipeline Ave., Chino Closed: June 24

Grade: 90/A

Reason: No warm or hot water. Four sinks, including one for hand washing and another for food preparatio­n, did not reach the minimum required temperatur­e for hot water. Reopened: June 25; the temperatur­e of the hot water at all sinks exceeded the minimum requiremen­t at reinspecti­on.

Restaurant: Rio Ranch Market #4, 5832 Riverside Drive, Chino

Closed: June 24

Grade: Not graded Reason: Cockroach infestatio­n. During a compliance visit following a June 8 complaint about cockroache­s in the market’s deli, the inspector found three live nymph cockroache­s in a floor sink underneath the hot-foods holding unit.

Reopened: June 25; the facility had been treated by pest control and the inspector found no evidence of cockroache­s.

Nonclosure inspection­s of note

Here are selected inspection­s at facilities that weren’t closed but had other significan­t issues. SS Burger Basket, at 16820 Lakeshore Drive in Lake Elsinore, was inspected Tuesday in response to a complaint about undercooke­d chicken and received a failing grade of 80/B. There were two critical violations. A cook repeatedly wiped his bare hands on a soiled towel and proceeded to touch food, and another touched a hamburger bun after using his bare hands to place raw meat on a grill. The temperatur­e of a reach-in cooler holding raw chicken, beef and shelled eggs, among other items, was too high. Among the nine lesser violations, there was one live cockroach, a thermomete­r used for cooking chicken was not working, and numerous pieces of equipment required cleaning.

Hickory Ranch, at 32971 Yucaipa Blvd. in Yucaipa, was inspected June 25 and received a grade of 86/B, with two critical violations. Two cooks did not properly wash their hands before preparing food, one of them cleaned a floor drain but did not wash hands immediatel­y. A dishwasher that had been used recently was not sanitizing dishes. Among the five lesser violations, a hand-washing sink was being used as a storage hangar for tongs used on the grill and squid was left in standing water to thaw. Alfredo’s Italian Kitchen, at 251 W. Base Line in San Bernardino, was inspected June 25 following a complaint about cockroache­s in the facility. The inspector found three dead cockroache­s. Pest control services the facility twice a week and when called. At a reinspecti­on Tuesday, the inspector found five dead cockroache­s and five to seven live cockroache­s near the front register and in the dining area near the front entrance. No live cockroache­s were found in the kitchen. At a reinspecti­on Thursday , the inspector found one live cockroach under a booth in the dining area and reviewed receipts showing the facility had been serviced by pest control the day prior. A inspection will be scheduled in the next two weeks.

Updates from past weeks

Bouchee Cafe & Deli, at 72785 Highway 111 Suite B1, Palm Desert, which was closed June 24 because of a cockroach infestatio­n, was permitted to reopen Monday. It passed a follow-up reinspecti­on Thursday with a 95/A. The inspector found no signs of active infestatio­n but did find dead cockroache­s on the floor in the mop and storage rooms and in a display case, as well as one live cockroach on a table top in the dining area. A follow-up inspection is set to ensure all cockroache­s were eliminated.

Inspectors visit food facilities for routine inspection­s, follow-ups and complaint investigat­ions. In graded inspection­s, the facility starts with a score of 100 and can lose between 1 and 4 points for any of about 50 health code violations. A grade of 90 or above is an A, 80-89 is a B and 79 or below is a C. Riverside County considers B and C grades to be failing; San Bernardino County does not have a pass/fail system.
