Local provider wins national recognition
Community Health and Dental Care gets patient services award
Community Health and Dental Care Inc. has achieved Patient-Centered Medical Home Recognition from the National Committee for Quality Assurance (NCQA). The agency was recently recognized for demonstrating practices that follow medical evidence to deliver better care, lower costs, and higher patient satisfaction.
“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” Margaret E. O’Kane, NCQA president, said in a press release, adding that Community Health and Dental “has the tools, systems and resources” to provide its patients with the right care.
The program was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts. It reflects the input of the American College of Physicians, American Academy of Family Physicians, American Academy of Pediatrics and American Osteopathic
Association and others, according to the release.
The standards “emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement,” the release stated.
Community Health and Dental Care provides medical, dental, OBGYN, vision, podiatry, physical therapy, medication assistance therapy (MAT), integrated behavioral health, care and case management and free transportation to and from all of its locations, including: 351 W. Schuylkill
Road in North Coventry; 700 Heritage Drive in Lower Pottsgrove; 802 Heritage Drive in Lower Pottsgrove; and its newest location, 1315 Route 100 North in Washington
Township.
