Todd Frazier hoped to do something special in his home debut for the New York Yankees. Frazier definitely did that Tuesday — he grounded into a triple play. The previous record of 1:04.35 was set by Lithuania’s Ruta Meilutyte. Ledecky breezed to her third gold medal of the meet, capturing the 1,500 freestyle by more than half the length of the pool.
