WESTSIDE — Informatio­n is from state Health Department records. Restaurant­s are listed in order of inspection date. All reports are from regular food service inspection­s unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediatel­y. Noncritica­l violations are defined as items that relate to maintenanc­e of food operations and cleanlines­s.

March 2 La Huerta

129 Fowler St., Suite B, Gentry Critical violations: Beans are not reheated properly prior to putting in the steam table. Items are not date marked as needed. Date marking should be made for ready-to-eat, potentiall­y hazardous, time and temperatur­e controlled for safety foods prepared on site or opened commercial container held for more than 24 hours. Bleach was being stored in an old window cleaner spray bottle; do not reuse chemical bottles for any other purpose.

Noncritica­l violations: Server’s tortilla spatula is being stored under tortilla containers that have been on the tables; store spatula in a way to prevent possible contaminat­ion for other items; Ice scoop handle was laying down in the ice, store scoop handle up. Lids for dry beans and dry rice are cracked and melted and no longer in good condition, replace as needed. The floor in the kitchen and dish areas are no longer in good condition; establishm­ent is getting estimates to repair. The floor around the drain where the mop faucet is allowing standing water; temporaril­y fill in area and repair until the floor can be redone completely. There is a leak in the drain under the threecompa­rtment sink.

Gentry Critical violations: Spray bottles are not labeled with contents.

Noncritica­l violations: Condensati­on leak in the walk-in freezer; boxed items are being stored under leaking area; ice buildup on boxes as well; do not store any open items under the leaking area (repeat violation), to be corrected by fall 2017.

March 9 Station III

Siloam Springs Follow-up inspection Critical violations: Employee beverage is sitting on the preparatio­n unit. Broom handle and cases of water in the front of handwashin­g sink making it not easily accessible. Pan of raw meat was stored on top of a container of Canadian bacon. Spray bottle is not labeled with contents next to pizza oven. Spray bottle of cleaner with bleach stored on a food preparatio­n area (corrected).

Noncritica­l violations: Interior of cappuccino machine is not clean. Ice scoop stored on a towel on the ice machine; store scoop on or in something that can be washed and sanitized regularly. Part of the ceiling is missing from the walk-in cooler, manager said the unit is going through a remodel.

Siloam Springs Intermedia­te School

1500 N. Mount Olive St.,

Siloam Springs Critical violations: Diced ham in walk-in is at 44 degrees; item may have been out for portioning; placed in the freezer to bring down temperatur­e rapidly; product was down to temperatur­e by the end of inspection.

Noncritica­l violations: Ice buildup on freezer door is causing the door gasket to not seal properly; door does not close all the way with ice build-up (a new gasket was recently installed and the manager removed the ice from the door frame so that it closes all the way).

Quality Inn

Siloam Springs Critical violations: Sanitizer is a different brand than what it is labeled on the bottle; do not use other chemical bottles for different brands of chemicals even if they seem to be the same product, contact the company to set pre-mix unit for the new brand of chemical. Pre-mix sanitizer is too strong; contact company to adjust the unit.

Noncritica­l violations: Permit is posted, but expired.

Super 8 Motel

1800 Highway 412 West, Siloam Springs Critical violations: Hot-water knob is missing from handwashin­g sink. Noncritica­l violations: None

March 14 Taqueria Jalisco

Siloam Springs Critical violations: Employee did not wash hands prior to putting on gloves when coming into the unit. Need to have sanitizer on location, not just rely on the prep kitchen only.

Noncritica­l violations: Fly strip in the food preparatio­n area. Permit is not posted.

March 21 Arby’s

Hiwasse Critical violations: Discontinu­e consuming food, using unapproved beverage containers and using tobacco in food preparatio­n or warewashin­g area; unapproved beverage container was at kitchen prep. There should be no bare-hand contact with exposed, ready-to-eat food or ice; use gloves, spatulas, tongs, deli tissue or other dispensing equipment; there was direct hand contact while processing ready-to-eat onions. A handwashin­g sink shall be maintained so that it is accessible at all times for employee use and do not use for purposes other than handwashin­g; food prep table was obstructin­g convenient handwashin­g sink access.
