Horizon Health and Wellness receives Joint Commission certificates
Horizon Health and Wellness has earned certifications from The Joint Commission for ambulatory health care, behavioral health care and as a primary care medical home.
The certification demonstrates the focus of Horizon Health and Wellness on improving care coordination across the continuum of care, particularly as it relates to information sharing, transitions of care, hand-off communications and other key activities impacting patients moving between the hospital and outpatient care settings.
The Joint Commission is the most prestigious United States based organization that accredits more than 21,000 U.S. health care organizations and programs. The international branch accredits medical services from around the world. Earning the certification is an achievement of great regard, the company said.
Horizon Health and Wellness underwent a rigorous onsite review to assess its compliance with certification standards for each area in which Horizon Health and Wellness was certified. During the review, The Joint Commission experts evaluated several areas, including communication, information technology integration, patient and family engagement, service integration, and quality improvement activities that span the continuum of care.
The award of the certifications proves that Horizon Health and Wellness has successfully carried out the integration of patient care between hospitals and ambulatory health care providers and organizations in Southern Arizona. It also provides a pathway for leaders to improve patient satisfaction and reduce preventable admissions and use of emergency services.
“We are delighted to be accredited by The Joint Commission, the premier health care quality improvement and accrediting body in the United States,” said Norman E. Mudd, CEO of Horizon Health and Wellness. “By achieving these certifications, we are fostering improved collaboration and patient care, while also helping to reduce the risk of adverse events.”
